What Is Supported Living?

Supported living is a model of care and housing that helps people with disabilities, mental health needs, or complex conditions live as independently as possible in mainstream accommodation.

Unlike residential care homes, service users have their own tenancy and greater control over their daily lives, with support tailored to individual needs.

Key features include:

  • Individualised support plans focused on goals, strengths and preferences.
  • Assistance with daily living skills and household management.
  • Community engagement and social participation.
  • Flexible support levels from a few hours to full-time assistance.

Who We Support

House 21 Supported Living is suitable for adults, including, but not limited to, those with:

Working with Individuals Affected by Substance Misuse

At House 21, supporting people who are overcoming addiction. Whether drug or alcohol related, it is a core part of the our mission.

House 21 provides a person-centred, therapeutic environment aimed at helping individuals stabilise their lives, address the underlying issues linked to substance misuse, and build sustainable pathways towards lasting recovery.

This includes developing tailored care plans, daily living support and strategies to reintegrate into community life in a positive, structured way. We recognise that each person’s journey with addiction and mental health challenges is unique and requires a holistic response.

A critical aspect of this work has been the close collaboration with The Forward Trust, a well-established UK charity specialising in drug and alcohol support, treatment and recovery services

Forward Trust delivers a broad spectrum of evidence-based interventions across community and custodial settings, helping individuals to either reduce harm or work towards complete abstinence and sustained recovery from substance misuse.

Together, House 21 and Forward Trust support clients through coordinated care pathways that maximise opportunities for recovery, including:

  • Individual assessment and goal-setting
  • Therapeutic and psychosocial support
  • Access to specialist treatment options
  • Recovery communities and peer networks

This collaborative approach emphasises that lasting change often requires both personal commitment and professional support, with House 21 focusing on stabilisation, personal development and reintegration, and Forward Trust bringing specialised addiction recovery expertise.

Combined, these services help individuals not only abstain from substances but also build supportive routines, relationships and life skills that contribute to long-term wellbeing and social inclusion.
